AFP Community Consultation Survey

Age-friendly Peterborough (AFP) is developing its 2023-2025 Community Action Plan for the Peterborough region, and we want to hear from you!

This survey is intended for older adults (ages 50+) who reside in the greater Peterborough area, which includes the City of Peterborough, Peterborough County, Hiawatha First Nation and Curve Lake First Nation. The information you provide in this survey will be used to gauge how age-friendly the Peterborough region is today and what could help make it more age-friendly in the future. Results from this survey will be published in the AFP Report Card and will be available June 2022.

This survey will take approximately 20-30 minutes to complete. Your participation is voluntary, and you are welcome to answer as many or as few questions as you would like. Personal information will remain confidential. Please base your responses on your own personal experience. If you provide care to older adults, you can also help them complete their own survey.

All information is collected in accordance with the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA). Personal information may be subject to access to information requests under MFIPPA. Questions about the collection of information should be directed to Jayne Culbert, Age-friendly Coordinator, at 705-748-8830 ext. 3227 or at: jculbert@peterborough.ca.

Completion of this survey implies that you consent to participate.
If you require assistance completing the survey, or would like to request it in an alternative format, please contact 705-748-8830 ext. 3624 or afpadmin@peterborough.ca
